If you are unable to complete and print these forms prior to your visit, please arrive at least 30 minutes prior to your scheduled appointment to complete paperwork in the office. If you have extensive medical history to document, we recommend arriving more like 45 minutes early.

Imaging Policy
Please bring all imaging on CD-ROM to your appointment to ensure continuity and quality of care. Present the discs at check-in. If your visit includes imaging review, arriving without them may result in cancellation and rescheduling. Thank you for your cooperation.
*If your imaging is from Bowes Imaging Center (BIC), Lakewood Ranch Medical Center, Manatee Diagnostic Center, Manatee Memorial Hospital, or SimonMed, please note that you are exempt from this practice of obtaining your images on a CD for us.
Cancellation Policy
If you need to cancel your appointment, we ask that you do so 24 hours in advance. Failure to do so may result in a $50 no-show fee.
