If you are unable to complete and print these forms prior to your visit, please arrive at least 30 minutes prior to your scheduled appointment to complete paperwork in the office. If you have extensive medical history to document, we recommend arriving more like 45 minutes early.
At this time, we are asking for all imaging to be brought along to appointments on CD-rom discs. This is for continuity and quality of care. Please have your discs and present them at check-in to the office. If your appointment consists of reviewing any imaging, arrival without discs renders your appointment subject to cancellation and rescheduling. We thank you for your cooperation and understanding.
Please bring your picture ID, insurance card, and medication list (with dosages).
If you need to cancel your appointment, we ask that you do so 24 hours in advance. Failure to do so may result in a $50 no-show fee.
